Membership Terms
Renews Yearly
- Valid for one year from enrollment and renews annually unless canceled before renewal
- Fees are non-refundable and membership is non-transferable
- Cannot be combined with dental insurance, other discount plans, coupons, or promotions
- Not valid for treatment covered by insurance or for outside specialists or labs
- Invisalign patients must remain active members to receive the discount
- Valid only at Hamilton Family Dentistry
